Important: If you have been the victim of identity theft through the theft of your credit card data, please call your credit card company and have your credit card numbers changed, or your account closed. Please do not continue until you have done so!
Note: This process is completely anonymous. We will NOT ask you for your new credit card number. You will NOT be required to create an account with us. We will NOT ask for your address. We do NOT need your E-Mail address unless you wish to be notified that the information you provided was helpful to us.
You will need the following to continue:
- The exact Date of the FIRST fraudulent charge on your credit card account.
- The first six numbers of the credit card which was stolen (the card which should no longer be valid because you had the account deactivated)
- The name of your bank or credit card company who issued the card.
- Six (6) months of credit card statements immediately prior to the first fraudulent charge.
In the following Credit Card Theft Report, we will ask that you provide the Internet URL (Ex: http://www...) of the Internet Business where you made a purchase with your credit card. Please use your credit card statements as a reference to where you've used this card, and the date you made the purchase.
We will NOT ask you what you purchased, or how much you spent.
